since when is an accepted holding penalty ADDED to the preceding play? Shouldn't it either be declined (keep the outcome of the play) or accepted and have the yardage marched off with a repeat of the preceding down?

Does anyone recall seeing a holding call against the Gints that was NOT between the 35 yard lines and/or outside of three minutes in a half?
